This Head of Service role has a primary focus on hospital discharge and the

Council’s relationships with health colleagues. We aim to facilitate timely discharge,

achieving the best outcomes for people in Cambridgeshire, and supporting

them to live at home, in their communities for as long as possible. You will

report to a Service Director in our Adults, Health and Commissioning

directorate where you will have a supportive peer group of Heads of Service

covering various service areas.

We work on a hybrid basis and expect the post-holder to be in the office at least two days per week. We have offices at New Shire Hall in Alconbury Weald, Hereward Hall in March and other locations; we are happy to discuss the most suitable option with the successful candidate.

What will you be doing?

You will provide leadership to the Transfer of Care Service and other adult

social care functions, as needed. You will be highly visible across the wider

health and social care system, developing and maintaining key relationships to

drive forwards our ambitions to work closer to communities with a ‘home first’

approach. You will lead operationally as well as strategically to ensure that

the service is safe and effective while always driving forward improvement,

including influencing wider system change. You will ensure your services are

ready for CQC assurance and aspire to the highest of practice standards at all

times

About you

You will be an authentic leader with social work values at the centre of all

you do. You must be able to quickly understand and work within the wider

system priorities and complexities, using your skills to navigate barriers and

maximise opportunities to drive towards ever improved outcomes for the

people the service supports. You will operate with absolute clarity about

adult social care’s role and responsibilities, including the boundaries around

those and where the responsibility of other partners sits. You will be

committed to leading a diverse and inclusive service where all colleagues

feel supported and have a voice, and you will demonstrate this at all time

in your own behaviours.
