The service you will work in….

The aim of the post is to deliver a tailored service to house rough sleepers. The postholder will work with the rough sleepers to develop, support and maintain independent living skills and general wellbeing and maximise the  independence of people with history of rough sleeping with the final aim to enable positive move on to prevent rough sleeping.

A day in this role looks like…

As a Tenancy Sustainment Coordinator at Hestia, you will play a crucial role in our Mental Health and Complex Needs Service. Your contributions will directly influence the lives of our service users, guiding them towards a path of empowerment and independence.

A day will never look the same and some of the things you can expect to do/be responsible for are:

  • Deliver a tailored service offer to previous rough sleepers and those at risk of rough sleeping.
  • Develop, support and maintain independent living skills and general wellbeing and maximise the independence of people with history of rough sleeping.
  • Effectively and professionally work with the referring agents at the H&F PATHS Team.
  • Enable positive move on to prevent rough sleeping.
  • Co-produce needs, risk and aspiration based assessments with people using the service to ensure the delivery of a personalised service.
  • Assess and support service users with a range of needs, risks and aspirations for example rough sleepers with mental health and substance misuse issues, offender backgrounds who have experienced complex life experiences.
  • Formulate, implement and review personalised, client-focused SMART support plans to develop, review and achieve goals and aspirations.
  • Engage service user's supporters in their support plans (with consent) including statutory and voluntary sector providers.
  • Utilise key-working sessions with key clients to enable them to develop, sustain and maximise independence, maintain their accommodation and enjoy a good quality of life.
  • Deliver a flexible, dynamic, proactive and responsive service according to client needs including work outside of ordinary offices hours including weekends and evenings.
  • Motivate service users in all aspects of tenancy sustainment.
  • Engage service users in activities which support their physical and mental wellbeing including meaningful day time activities and maintaining healthy relationships.
  • Enable the service users to take responsibility for their health needs including physical health and mental health.

  • Can demonstrate knowledge and experience of working with welfare benefits legislation (including welfare reform) and (Health and Safety).
  • Can demonstrate knowledge and experience of working with the causes of homelessness and the ability to create interventions with service users to address these.
  • Excellent customer service skills.
  • Impeccable organisational skills.
