As a Community Matron, you'll play a pivotal role in the delivery of our Integrated Neighbourhood Teams (INT's) across West Lancashire, working alongside multiple internal and external services.

Your expertise and compassion will make a significant difference in improving the health outcomes and quality of life for individuals under your care.

You will need to be an experienced practitioner, with proven clinical abilities in the management of patients with complex health needs and long term conditions in the prevention of avoidable hospital admissions.

Main Responsibilities
  • To work autonomously as a senior practitioner In managing the acute and complex health needs of patients with long term conditions and frailty.
  • To act as the lead clinician coordinating personalised management plans to reduce the risk of avoidable hospital admissions.
  • To deliver clinical assessment, Initiate and manage diagnostics, confirm diagnosis, and recommended clinical management plans that Include prescribing If clinically Indicated, according to personal and professional competencies.
  • To be practice as a core member of the Integrated Neighbourhood teams.
  • To deliver responsive care when patients report signs of exacerbation or deterioration of their long-term condition and where clinically Indicated complete same day urgent assessment.
  • Provide clinical leadership and role modelling for junior members of the team and other members of the wider multi-disciplinary team. 
  • To provide Community Service representative at Primary Care Network MDTs and develop positive professional relationships and networks to enable coordinated and multi-disciplinary approach to supporting patients with complex health and social care needs.
  • To case manage identified patients who require intensive clinical support, aiming to prevent avoidable hospital admissions, promoting self-management and self-empowerment to support patients in the long-term management of their condition.
  • Provide health education; provide regular support and reviews as required.
  • The post holder will be able to recognise the early symptoms of disease exacerbation, acute illness based on an understanding of the chronic diseases, the disease process, clinical assessment skills, current evidence, and practice standards.
  • To provide caseload leadership and management that demonstrates quality, safety, and productivity in line with commissioning intentions.
  •  Act as a role model for the delivery of high standards of care that complies with NMC codes of conduct and Trust policies.

The Ideal Candidate

Essential

  • Degree in appropriate subject or evidence of education to degree level.
  • Current NMC registration
  • Master’s degree (completed or working towards), or equivalent work-based experience.
  • Clinical examination and clinical diagnostics (advantageous) or willing to work towards.
  • Registered Non-medical prescriber (advantageous) or willingness to undertake the programme.
  • Post registration qualification, or demonstrable equivalent experience.
  • Evidence of continued professional development (CPD)
  • Evidence of clinical experience at a high level in a range of clinical settings including the community / primary care.
  • Knowledge of chronic disease management.
  • Evidence of influencing, motivating, and negotiating with others to achieve change in relation to care.
  • Experience of clinical leadership and clinical pathway development
  • Understand and experience of successful multi-agency working.
  • Awareness of current national and local agenda in health and social care
  • Evidence of being able to communicate complex, sensitive information and advice on healthcare to patients/carers/colleagues.
  • Ability to network with multi-disciplinary colleagues.
  • Awareness of factors that contribute to good health and the importance of promoting these in line with the public health policy.
  • IT literate – with strong report writing skills.
  • Ability to work under pressure and manage a diverse workload.
  • Excellent communication, organisational and interpersonal skills
  • Risk assessment skills.
  • Ability to understand and analyse complex data.
  • Self-management and motivation skills
  • Confidence to challenge poor practice and ability to address difficult issues.

Desirable

  • First line management experience

 

Other requirements:

  • Ability to travel and work across boundaries when/if required.
