We’re looking for skilled and dedicated Nurse to join our North Kent Rapid Response Service with a base at Sheppey Community Hospital.

You'll help us to deliver a high standard of care for our patients out in the community, and to continue to help improve and develop our service by joining a multidisciplinary team made up of  Nurses, Occupational Therapists and Physiotherapists.

This is an exciting opportunity to be involved in creating an efficient service focusing on admission avoidance and supporting early discharge in line with the NHS 10 year plan.

The Rapid Response team works across 7 days a week from 8am - 8pm.

Main Responsibilities

As a Nurse within HCRG Care Group Swale rapid response team you’ll join a team who have recently been through transformation to create a fully multidisciplinary 2hr response service. 

Your days will be fast paced and varied and include offering support to patients and families whom are in crisis or acutely unwell, in order to prevent a hospital admission.  Also supporting early hospital discharges by offering clinical assessment and treatment within their own home as opposed to a lengthy acute hospital stay.  Advanced clinical assessments skills will be greatly utilised within this rapid response role.

The Ideal Candidate

You will have a degree qualification and registration with relevant body (NMC) for this role.  HCRG Care Group have expansive training opportunities including apprenticeships available for staff development. The team also have a robust in-service training schedule, where staff enjoy sharing their knowledge and skills with others, outside agencies also attend to upskill our workforce as part of our continuing professional development. 

You will have a full UK driving license with access to a car for work purposes.

You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
