We are looking for a permanent Band 5 District Nurse to join our friendly, inclusive and forward thinking District Team, working across the West Lancashire area to provide care and support to patients in community settings.

To work as part of the 24-hour district nursing service, under the direction of the District Nursing Team Manager / Leader, providing high quality holistic care to patients.
The District Nurse role carries delegated responsibility for the assessment, planning, implementation and evaluation of evidence-based nursing to patients in their own homes.
To assume delegated responsibility for the supervision of the team in the absence of the District Nursing Team Leader, when required.

You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
