As the Matron of the Diagnostic and Treatment Centre (DATC) you’ll be working as part of the community Partnership with Frimley NHS system in Farnham. DATC deliver a variety of services to the local population of Northeast Hants and Surrey, services provided include a one stop style assessment for older adults called a Comprehensive Geriatric assessment. Our aim is to improve patient outcomes, quality of life and support with anticipatory care needs.
DATC have a multidisciplinary team that work together to provide a holistic assessment of patient’s needs. We work collaboratively with our partners in the wider MDT such as GPs, acute hospital services and community HCP's. We have access to a variety of diagnostic investigations.
We also provide a Palpitations pathway, offering 24 hour and 7-day ECG monitors to patients aged 18 and over.
We are also proud to deliver a therapy lead Parkinson group and will shortly be introducing a therapy lead falls group.
This is the perfect opportunity for any Band 5 Nurse who has either in the community, rapid response or in an acute trust that are looking to take the next step.

As a staff nurse you will develop strong team working within the wider Diagnostic and Treatment Centre , liaising with the Administrator and Clinician.
You will have a nursing diploma/degree and hold a valid NMC registration. Minimum 3-year post reg experience and previous community experience would be desirable, but not essential.
