An exciting opportunity has arisen, to appoint an Associate Community Matron based in Bath and North East Somerset!
As an Associate Community Matron you would work closely with the Matron, Primary care providers and other members of the Multidisciplinary team to support patients in the community setting who are vulnerable and have complex healthcare needs. You will case manage your own group of patients, with the support of the Matron, providing clinical assessments and intervention for adults in the community.
This is a flexible part-time position which includes potential evening or weekend work to align with the Urgent Care Response model.
