Playing an integral role within our Urgent Community Response Team (UCR), which aims to ensure crisis response care is available to all people within their homes or usual place of residence, including care/residential homes, within two hours, the post holder will be the lead clinician in the assessment of patients’ holistic needs and will devise a clear clinical management plan.

This service operates over a seven-day rota, 365 days a year and is operational between the hours of 8.00 am – 8.00 pm, 7 days per week.

Main Responsibilities

You will aim to facilitate a period of stabilisation of the patients’ condition to prevent the urgent need becoming an emergency need and supporting patients to restore or maintain their independence in the community. In order to achieve this your main responsibilities will include:

  • To assess and identify the individual needs of referred patients and their carers, planning and implementing appropriate treatment programmes
  • Respond to urgent requests for patient home visits where the focus of the visit is admission avoidance
  • To record and report findings appropriately following the standard procedures used by the service to maintain high levels of written / verbal communication
The Ideal Candidate

We are seeking a friendly and ambitious candidate to join the welcoming culture of our UCR team. You will have the following:

  • Current and valid HCPC registration
  • Diploma / degree in Occupational Therapy
  • Excellent communication skills, ability to share information at MDT case conference
